Tobacco Program Overview

The Colorado Clinical Guidelines Collaborative (CCGC) Tobacco Program is an integral part of the work that CCGC is doing to improve healthcare in Colorado.

Funders

The CCGC Tobacco Program is funded by grants from the following. Click on a name below to be taken to their website.

STEPP Grant

Funding to support Colorado's tobacco prevention and control programs is made available through the passage of Amendment 35, the tobacco tax ballot initiative passed in November 2004 and HB05-1262, the enabling legislation for Amendment 35 signed into law on June 2, 2005.

With funding from STEPP, CCGC will increase the incidence of healthcare provider advice and intervention with tobacco users using three related and interdependent activities:

1. CCGC staff will serve as clinical experts in tobacco control to educate the Colorado medical community using a standard set of Colorado-focused tools and resources based on the Treating Tobacco Use and Dependence Clinical Guideline and other evidence.

2. CCGC staff will work with individual clinical practices to educate providers on the tobacco guideline (5 A's), resources, and assist them in a multi-step long-term process of re-desiging their practices to incorporate the 5 A's and tobacco identification and reporting systems.

3. CCGC staff will provide technical assistance and funding to organizations that have access to networks of clinics and can facilitate the change necessary within these unique networks to identify tobacco users at each patient encounter and provide counseling/referrals on a routine basis.

Caring for Colorado Foundation and The American Legacy Foundation Grant

CCGC is the recipient of a grant with matched funds from the Caring for Colorado Foundation (CFC) and the American Legacy Foundation.  This grant allows CCGC to build and expand upon our scope of work funded by the state health department.

With funding from CFC and Legacy, CCGC will:

1. CCGC staff will continue to work with clinical practices through tobacco systems redesign methodology expanding upon work funded by STEPP.

2. Explore ways of improving communication between providers and patients following their office visits regarding tobacco use and cessation by using tools such as registries, secure automatic email reminders or automated phone calls.

3. Explore cost effective approaches to communicating with healthcare providers to increase their awareness of cessation resources, such as sending brief video clips via email.
